Information we collect on this website

When you complete a form on this site, we collect the details you give us: your name, email address and, if you choose to provide it, your phone number and message. We use these only to reply to you and to arrange your appointment.

We do not sell your information, and we do not share it with third parties for their own marketing.

This website is not a secure channel for health information. Please do not use the form here to send medical history, symptoms, diagnoses, treatment details or images. If you need to share something clinical, call the practice on 970-816-6118 or fax 970-807-0052, and we will record it in your patient file where it is properly protected.

The site is deliberately built so that it never collects, stores or transmits protected health information. The contact form asks only for a name, an email address, a phone number and a short message, and nothing on this site is connected to your dental record. Appointment forms are downloaded, completed on paper, and returned to the practice in person or by fax rather than submitted online.

Your health information (HIPAA)

You have the right to inspect and request a copy of your record, to ask us to correct it, to request restrictions on how it is used, to ask for confidential communication, and to receive an accounting of certain disclosures.
